Mona and Family wish to sincerely thank all relatives and friends for their kindness and many expressions of sympathy, and for the generous donations received for the Parkinson's Disease Society and SANDS in their recent sad loss. Special thanks for the compassionate care given to Jack by everyone involved, to the Doctors, District/ Community Nursing Team and the Home Care Team, but particularly the Staff at The Calderdale Royal Hospital on Ward 5C and the Pastoral Care Team. We are especially grateful to the Reverend Steph Jenner for her comforting Service and to Jared Barker and the staff at Co-Operative Funeralcare in Brighouse for their care in making and carrying out the funeral arrangements.
