Loving and devoted Father of Samantha, Kelly, Conor and Madeline and a dear Brother of the late Chris.

Sam will be missed greatly by all who knew him.

The Funeral Service and Committal will take place at 2.45 p.m. on Friday 17th April at Macclesfield Crematorium.

Family flowers only, by request, but donations may be made, if so desired, in favour of the East Cheshire Hospice or the MS Society.
